Establishment and Installation Heads: Statutory Requirement to Review and Post EHS/OSHA Form 300A Summary for Calendar Year 2011

The Occupational Safety and Health Administration (OSHA) requires all Postal Service™ installation and estab­lishment heads to prepare and post the Employee Health and Safety (EHS)/OSHA Form 300A, Summary of Work-Related Injuries and Illnesses, for calendar year (CY) 2011 (see page 23). This requirement complies with 29 CFR 1904 and the Employee and Labor Relations Manual, 821, Actions in the Event of Accident, Injury, or Illness.

Reviewing and Closing Out EHS/OSHA Form 300 Log

The electronically generated EHS/OSHA Form 300A is a summary of data appearing on the OSHA 300 Log. Before you complete and sign the EHS/OSHA 300A summary, you must ensure that all entries on the EHS/OSHA 300 Log are accurate and complete. Verify the completeness and accu­racy of the electronically generated EHS/OSHA 300 log. All information except for “employment information” and “sign here” will be automatically populated on the EHS/OSHA Form 300A.

To access the EHS/OSHA 300 and 300A documents, do as follows:

n Go to http://blue.usps.gov.

n Click My Work.

n Log on.

n Go to Safety Resources Tools.

n Go to Report an Accident or Incident.

n Select Print 300 or Print 300A.

n Select Work Area.

n Enter ZIP Code™ of installation.

n Select Start Search.

n Select Work Area ID.

n Select Calendar Year 2011.

n Select Execute.

Reviewing and Posting EHS/OSHA Form 300A

Installation or establishment heads are required to com­plete, review, sign, and post the EHS/OSHA Form 300A. When you sign on the Company Executive line, you are cer­tifying that you have verified that the entries on the EHS/OSHA Form 300A — and its supporting documents — are true, accurate, and complete.

The following steps are required for completing and posting the EHS/OSHA Form 300A:

1. Estimate the average number of all employees at your establishment or installation for the calendar year. Place this number of employees in the location designated as number 1.

2. Enter the installation’s CY 2011 total hours worked (exposure hours). Place these hours in the location designated as number 2.

3. As installation or establishment head, sign your name, and provide your job title, office telephone number, and date in the location designated as number 3.

4. Make a copy of the completed and signed docu­ment. File the copy in local files and post the original document where it can be viewed by all employees. Post on employee bulletin boards from February 1, 2012, through April 30, 2012. Document the date posted and date removed on the backside of the EHS/OSHA 300A. Retain this form for 5 years.

From February 1 through April 30, 2012, facility heads must post a completed and signed copy of the EHS/OSHA Form 300A for CY 2011 in a conspicuous place at every establishment where employees work or report to work. You must post an EHS/OSHA Form 300A even if you did not expe­rience any OSHA injuries or illnesses in the calendar year.

Note: You do not need to post the EHS/OSHA Form 300 log with the EHS/OSHA Form 300A. Pull down the original Form 300A after April 30, 2012, and maintain it in your file system for 5 calendar years.

Retaining Forms

Ensure that an EHS/PS Form 1769/301, Equivalent, is on file for each entry on the EHS/OSHA Form 300. Postal Service installations must retain EHS/OSHA Forms 300, 300A, and 1769/301 for 5 calendar years. PS Form 1769, U.S. Postal Service Accident Report, often used before completing EHS forms and in conjunction with the OSHA forms, also must be retained for 5 years.
