Revised PS Forms 8105-A and 8105-B

We have revised PS Form 8105-A, Funds Transaction Report, and PS Form 8105-B, Suspicious Transaction Report. This article explains the changes, tells you how to order the new forms, and describes how to submit com­pleted forms.

We made the following changes to PS Form 8105-A:

Discussion of Changes

The EIN and TIN are business equivalents of a Social Security number and may be used if the purchase or redemption of a money order is on behalf of a business.

We replaced “Alien Registration” with “Resident Alien/Permanent Resident Identification.” However, Alien Regis­tration is still a valid identification and may be accepted as a substitute for the Social Security number.

If a customer purchases or redeems money orders dur­ing the same day and the amount exceeds $10,000, the retail associate must enter the customer’s occupation on PS Form 8105-A.

“Other” type of identification must be a U.S. government-issued photo identification card, which includes federal, state, and local/municipal government IDs. The only exception is that the Mexican Matricula Consular photo identification is acceptable as “Other” identification.

We replaced the word “Redeemed” with the word “Cashed” because some employees were incorrectly interpreting “cashed” to indicate that the customer used cash to purchase the money orders.

We provided eleven boxes for money order serial num­bers to accommodate inclusion of the check digit.

We added the new sections (i.e., number sold and redeemed, and ZIP Code of office) for quality control purposes.

When completing PS Form 8105-A, please remember the following:

Form 8105-B, Suspicious Transaction Report

We made the following changes to PS Form 8105-B:

It is a violation of federal law to alert or inform the cus­tomer that you are completing a PS Form 8105-B. Under no circumstances may you warn the customer or indicate in any way that you intend to complete a PS Form 8105-B to report the customer’s activity.

Each PS Form 8105-B must indicate a reason for believing the transaction is suspicious. This can be accom­plished by checking at least one or any combination of boxes 1, 2, and 3 in the Comments section. If boxes 1, 2, and 3 do not apply, you must check box 4 and provide suf­ficient comments to clearly indicate why the transaction is suspicious.

Discontinuing Old Forms

Offices must discard/recycle all stock of previous edi­tions of PS Forms 8105-A and 8105-B after they receive their new stock.

Submitting Completed Forms

Offices must continue to send completed forms to the address on the bottom of the form on the same day the transaction is conducted.
