PUBLICATION 75 NOTICE
Publication 75, Mover's Guide, is printed and distributed three times a year - April, August, and December. Some Post Offices automatically receive a supply allocated for individual consumers and individual businesses that need to change their address. (Post Offices that do not automatically receive a supply can call the Material Distribution Center at 800-332-0317, option 1, then option 2, for a supply.)
Do not give large quantities of the Mover's Guide to organizations who then give the guides to their customers, for example, real estate agencies and apartment-guide organizations. Such organizations who are interested in obtaining Mover's Guide for their customers should pay for the guide by ordering a supply from Imagitas. Imagitas charges a nominal fee of $59.00 per box of 125 supplies of Mover's Guide. A one-box minimum order applies.
Advise private organizations that are interested in obtaining Mover's Guide to call Imagitas at 800-816-6837 or e-mail Imagitas at moversguide@imagitas.com.

LEVEL 13 AND BELOW POST OFFICES
Within the next 30 days, Safety Performance Management at Headquarters will be mailing a package to all Level 13 and below Post Offices. That package contains safety and health inspection checklists and instructions for completing an inspection.
Please follow the instructions contained in the cover letter. Postmasters/Installation Heads (PM/IHs), or their designees, must complete a safety and health inspection using the checklist within 30 days of receiving the package.
Those annual safety and health inspections are required by ELM 824, Safety and Health Inspections.
PM/IHs must:
• Post the results of the inspection and ensure the abatement of all deficiencies in accordance with the instructions.
• Certify completion of the inspection to the servicing safety office.
PM/IHs in Level 13 or below Post Offices should contact their servicing safety office if they need technical assistance or if they do not receive the package that includes the inspection checklists and instructions.

ALL POSTAL EMPLOYEES
There's never been a better time to buy tires, get an oil change or lube job, or make those long-overdue automotive repairs. As part of the agreement that makes Goodyear the exclusive supplier of new tires for Postal Service vehicles, current and retired postal employees can receive savings on their personal tire and automobile service needs.
Participating Goodyear retailers are offering all Postal Service employees big-time savings under the special Goodyear VIP Program. Discounts include:
• Lube, oil, and filter for $14.95 (most vehicles).
• 15 percent off automotive services.*
• 10 percent off Goodyear brand tires.*
*Percentage discounts are applied to sale, everyday, or retail prices - whichever is lowest!
Qualifying employees will also be eligible to receive a Goodyear VIP Program credit card (no annual fee). Employee benefits include deferred or interest-free billing for 90 days to help minimize the financial impact of tire replacement and automotive service repairs.
The Goodyear VIP Program began on January 15, 2002. Postal employees can receive the special VIP pricing simply by showing the VIP credit card, employee photo identification, or other proof of Postal Service employment.
Under the 10-year agreement, which began September 9, 2001, the Postal Service expects to purchase 235,000 new Goodyear tires each year to outfit its fleet of more than 200,000 trucks, vans, and delivery vehicles, which travel a combined total of more than 1.1 billion miles annually. The Postal Service expects to save at least 10 percent on the cost of tires each year over the life of the contract.
For more information about the Goodyear VIP Program, visit the Purchasing and Materials intranet site at http://blue.usps.gov (click on Headquarters, then Purchasing and Materials, then Goodyear VIP Program). Retired employees and those who do not have intranet access can obtain program information or a paper credit card application by mailing a self-addressed, stamped envelope to:

CORRECTION
The article "Revised Form: PS Form 1094, Request for Post Office Box Key or Lock Service" in Postal Bulletin 22066 (12-27-01, pages 73-74) listed the incorrect information for Touch Tone Order Entry and the incorrect Quick Pick Number.
• The correct Quick Pick Number for PS Form 1094 is 113.
• The correct information is "Call 1-800-332-0317, option 1, then option 2."
Corrected information is shown in bold in the revised article below.
PS Form 1094 (see page 28), Request for Post Office Box Key or Lock Service, has been revised to reflect the January 7, 2001, fee changes. In particular, a $10 lock replacement fee has been added under the "Reason for Request" section. The "Fee Calculation (For Additional Keys Only)" section has also been changed to $5. Included in the $5 fee is $1 for the key deposit and $4 for each additional key. Recycle old forms and replace them with the revised PS Form 1094. Supplies are available in the Material Distribution Center.
Remember that the primary box customer (box applicant) may request that the Post Office box lock be changed. To change the lock, the customer must first pay the applicable lock fee as described in Domestic Mail Manual (DMM) R900.19.0. Lock fees are charged for replacing keyed locks and combination locks and for resetting combination locks. Lock fees are not refundable. Customers may turn in Post Office box keys for the old lock and receive a refund of the key deposit. Two keys are provided with each new lock, with a refundable deposit for each key charged under R900.19.0. Customers may obtain additional keys for the new lock under R900.19.0.
You can order the revised PS Form 1094 by one of the following means:
• Touch Tone Order Entry: Call 1-800-332-0317, option 1, then option 2.
• cc:Mail: Send an F3Fill-completed PS Form 7380, MDC Supply Requisition, to MDC Customer Service at TOKS001L.
• Mail: Mail a complete PS Form 7380 to the MDC at the following address:
