Effective June 4, 2007, supervisors must use PS Form 4584, Observation of Driving Practices, when they observe the driving practices of Postal Service™ employees with driving responsibilities. The December 2006 edition of this form replaces the August 1984 edition. All previous editions of PS Form 4584 are obsolete and must be recycled/discarded.
When used correctly, PS Form 4584 can aid in identifying and eliminating driving practices that could lead to accidents and injuries. The form also provides an opportunity for positive recognition and reinforcement of defensive and safe driving practices. Detailed information about observation of driving practices is provided in Handbook EL-804, Safe Driver Program, available online and, from the Material Distribution Center (MDC), in hard copy.
PS Form 4584, formerly a four-part set, is a two-page form, available on the Postal Service Intranet and from the MDC. The form has been completely reformatted for ease of use, and it can be completed online.
