[Revise the fax number in the first sentence of the first para­graph to read as follows:]

The district manager of Customer Service and Sales (or designee) must provide immediate notice of any suspen­sion by fax to 202-268-0002.***

[Add two new sentences at the beginning of the first para­graph to read as follows:]
