Revision of PS Form 1216, Employee’s Current Mailing Address

Effective immediately, PS Form 1216, Employee’s Cur­rent Mailing Address, is revised to emphasize that employ­ees are required to provide current and accurate address information. The Postal Service™ must be able to contact employees quickly — to perform routine personnel func­tions and ensure employee safety and health.

The November 2008 edition of PS Form 1216 replaces the June 2005 edition. Please destroy or recycle all previ­ous versions of PS Form 1216.
