Revised Privacy Act Notice

Effective May 2009, Notice 70, Privacy Act Notice Human Resources Forms, has been revised to include the addition of two new notices.

A Privacy Act notice is required whenever an agency collects personally identifiable information (PII) about indi­viduals and that information is retrieved by name or other personal identifier. When the Postal Service™ collects and maintains PII from other government agency forms and the information is covered under a Postal Service system of records, the Postal Service is required to provide its own Privacy Act Notice. This requirement is met by providing Notice 70 with the other agency form.

Notice 70 has been revised and is now three separate notices: one notice for each primary functional area that uses other agency forms as follows:

n Notice 70, Privacy Act Notice Human Resources Benefits Forms.

n Notice 70-A, Privacy Act Notice Investigative and Verification Forms.

n Notice 70-B, Privacy Act Notice Office of Workers’ Compensation Program Forms.

The May 2009 editions of these forms replace all previ­ous editions. Prior editions of Notice 70 are obsolete and must be discarded.
