Effective immediately, PS Form 2066, Personnel Security Questionnaire Update, is revised to reflect changes in levels of background investigations for personnel security clearances, as found in the Administrative Support Manual (ASM) 272, revised October 1, 2012.
PS Form 2066 is now available on the Postal Service PolicyNet website:

n Under “Essential Links” in the left-hand column, click PolicyNet.
n On the PolicyNet page, click Forms.
