Effective August 22, 2013, use only the June 2013 version of PS Form 3189, Request for Temporary Schedule Change for Personal Convenience. All previous versions of PS Form 3189 are obsolete and should be recycled.
PS Form 3189 is used by employees to request a temporary schedule change for personal convenience. By submitting a properly approved form to his or her supervisor, the employee agrees (if the request is approved by the supervisor) to forfeit any out-of-schedule premium to which he or she would otherwise be entitled, during the period requested. The appropriate union official must agree to the temporary change before it is submitted to the supervisor for approval.
Several changes have been made from the previous version of this form.
1. The From Regular Schedule and the Change Schedule To sections were revised to allow for the listing of individual service days and SDO was changed to Scheduled Day(s) Off.
2. The employee’s SSN was replaced with the EIN.
3. Within the Supervisor’s block, Processing Date and PSDS Tech Initials were changed to Date Entered and Entered By (initials).
