PS Form 3849 Revision: Now Available for Ordering from the Material Distribution Center

PS Form 3849, Delivery Notice/Reminder/Receipt, has been revised to reflect USPS Package Simplification Changes. Modifications include the name change from Express Mail^® to Priority Mail Express^®. Firm Bill and Delivery Confirmation options have been removed, and an Adult Signature option has been incorporated. Additionally, USPS Tracking augments the heading Article Number.

Do not discard or recycle the old version of the forms. Continue to use current supplies of the old forms, PS Form 3849 dated September 2009, before switching to the revised version.

Revised PS Form 3849 is now available for ordering from the Material Distribution Center (MDC) in Topeka, KS. You can order PS Form 3849 through the U.S. Postal Service eBuy2 Ordering System under the MDC eBuy2 catalog or the Touch Tone Order Entry (TTOE) system:
