Effective December 11, 2014, the Postal Service™ is revising the following forms to include updated Periodicals reference numbers from Mailing Standards of the United States Postal Service, Domestic Mail Manual®:
n PS Form 3500, Application for Periodicals Mailing Privileges.
n PS Form 3510, Application for Reentry or Special Price Request for Periodicals Publication.
n PS Form 3510-M, Application for Additional Mailing Office for Periodicals Publication (Pending Applications and Mailing at Non-Postal One! Offices Only).
For PS Form 3500, we are also adding new checkboxes:
n Under the “Postmaster” section, we are adding a checkbox to identify whether the office is a Centralized Acceptance Post Office™.
n In both Part B and Part C, we are adding a checkbox to identify whether the application contains electronic copies requiring an audit of circulation by a certified audit bureau.

n Under “Essential Links” in the left-hand column, click on PolicyNet.
n Click on Forms.
