PS Form 5957 Revision: Requirement-By-Applicant Matrix

Effective August 2020, the Postal Service™ revised PS Form 5957, Requirement-by-Applicant Matrix, to note that the chairperson of the review committee and the selecting official are responsible for forwarding the documentation cited in section 743.422, “Requirement-by-Matrix,” and section 743.423, “Use of Notes,” to local services for retention in eCareer as part of the vacancy package.

In addition, a seventh entry has been added to the instructions page to indicate that PS Form 5957 and any interview notes must be included in eCareer as part of the vacancy package.

Revised PS Form 5957 is now available on the Postal Service PolicyNet website:

n Go to blue.usps.gov.

n In the left-hand column under “Essential Links,” click PolicyNet.

n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 5000–5999.

n Scroll down to the form entry and click the PDF icon.

The direct URL for the Postal Service PolicyNet website is blue.usps.gov/cpim.
