PS Form 2181-D Revision: Disclosure and Authorization for Consumer Reports and Investigative Consumer Reports

Effective February 2021, the Postal Service™ revised PS Form 2181-D, Disclosure and Authorization for Consumer Reports and Investigative Consumer Reports, to make both “The Summary of Rights” and the PS Form 2181-D disclosure into standalone documents. This change will have no impact on the way information is provided to applicants and employees.

Revised PS Form 2181-D is now available on the PolicyNet website:

n Go to blue.usps.gov.

n In the left-hand column under “Essential Links,” click PolicyNet.

n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 2000–2999.

n Scroll down to the form entry and click the PDF icon.
