Revised PS Form 3650 is Now Available

Effective March 2021, the Postal Service™ revised PS Form 3650, Request for Expedited Packaging for Business Reply Mail Use, to summarize the requirements for customers who request specialized packaging supplies for use as Business Reply Mail® (BRM) inserts.

Customers (mail service providers or end clients) are not authorized to use free USPS® Expedited Packaging supplies as BRM inserts for solicitation purposes unless they are willing to cover the cost of the Expedited Packaging supplies. The following Priority Mail® BRM envelopes are available for purchase:

n Priority Mail Flat Rate Envelope EP14B (10” L x 6” W).

n Priority Mail Flat Rate Envelope EP14F (12.5” L x 9.5” W).

USPS-supplied Expedited Packaging is intended for full postage use, and not as reply envelopes where a high percentage are never mailed or returned. If a customer would like to use the envelopes in this manner, the Postal Service now offers customers the option to purchase Expedited Packaging supplies.

The company’s representative must review PS Form 3650 for accuracy, print their name and title, then date and sign the form. The USPS sales representative must email a copy of completed and signed PS Form 3650 to expeditedpackaging@usps.gov for review and approval. Once approved, the customer will be contacted for payment information before the envelopes are shipped to them.

Revised PS Form 3650 is now available on the PolicyNet website:

n Go to blue.usps.gov.

n In the left-hand column under “Essential Links,” click PolicyNet.

n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 3000–3999.

n Scroll down to the form entry and click the PDF icon.
