PS Form 5659 Revision: Daily Passport Application (DS–11) Transmittal

Effective January 9, 2022, the Postal Service™ revised PS Form 5659, Daily Passport Application (DS-11) Transmittal, to reflect updated information regarding the fee for overnight service from the Department of State.

USPS® facilities can use the following information to order PS Form 5659 through eBuy+:

The January 2022 edition of PS Form 5659 (which replaces the December 2021 edition) is available on the PolicyNet website:

n Go to blue.usps.gov.

n In the left-hand column, click Essential Links, and then click PolicyNet.

n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 5000–5999.

n Scroll down to the form entry and click the PDF icon.
