New PS Form 42, Declaration of Voluntary Resignation to Enter Military Service for Career and Non-Career Employees, is Now Available

Effective July 2022, the Postal Service™ discontinued form USPS 41, Resignation to Enter Military Service Declaration, and replaced it with new PS Form 42, Declaration of Voluntary Resignation to Enter Military Service for Career and Non-Career Employees. The new form can now be used by both Career and Non-Career Employees.

New PS Form 42 is available on the PolicyNet website:

n Go to blue.usps.gov.

n In the left-hand column, click Essential Links, and then click PolicyNet.

n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 0–999.

n Scroll down to the form entry and click the PDF icon.
