The notice may be printed on a sheet and handed to the customer’s representative or the notice can be read aloud to the representative.
Hard Copy Forms
The written notice must appear on or adjacent to the form close to where the information is collected. Alternatively, a separate written notice can be provided.
Telephone
Provide callers with a notice using an automation system or by providing the notice orally.
Online/Social Media
Provide notice on a portion of the page that is close to the area where the information is being collected. If such placement is not feasible or desirable, a pop-up box may be used that contains the text of the notice.
Email
Provide notice in the same email that solicits the information or other communication that precedes the collection of the information.
