Effective January 22, 2012, PS Form 1509, Sender’s Request for USPS Package Intercept Service, has been revised for use with the Postal Service’s new Package Intercept™ Service, which replaces the Recall of Mail service.
Changes to PS Form 1509 include the following:
n Renamed from Sender’s Application for Recall of Mail to Sender’s Request for USPS Package Intercept Service.
n Requests the required information about item to be intercepted, such as class of mail, shape of the item, and extra service tracking number.
n Upon signature by the applicant, confirms the applicant’s understanding that the Package Intercept service fee is nonrefundable; there is no guarantee that the item can be intercepted before delivery; and the item may incur additional postage due upon delivery to the sender.
n Provides detailed instructions for completing the form.
