The Barcoded PS Form 3533, Application for Refund of Fees, Products, and Withdrawal of Customer Accounts, previously found on Blue under Essential Links>Forms has been temporarily removed for revision.
If a retail unit does not have the barcoded PS Form 3533:
n Access PS Form 3533 by clicking the following link: http://blue.usps.gov/accounting/ and then clicking on Barcoded PS Form 3533.
n Non-Barcoded PS Forms 3533 submitted to the Scanning and Imaging Center (SIC) will be returned. This includes forms submitted with all zeros.
n Order PS Form 3533 from:
n The Material Distribution Center by calling
800-332-0317, select Option 3, then 2, then 1, and follow the prompts; or
n eBuy2 — Item Number/PSN 7530-01-000-9932.
Note: Only order small quantities. This form will be revised again in the latter part of 2014.
