Effective immediately, the Postal Service™ has revised PS Form 2181, Authorization and Release, to update the Privacy Act statement and to revise the language regarding the duration of the consent. This form is used for employees and contractors.
Please discard previous versions of PS Form 2181 and begin using the current form immediately. Revised PS Form 2181, Authorization and Release, is available on the Postal Service PolicyNet website:

n In the left-hand column under “Essential Links,” click PolicyNet.
n Click Forms.
