Effective May 1, 2018, the Postal Service™ revised PS Form 2489, Self-Identification of a Physical or Mental Disability, to ensure compliance with new Equal Employment Opportunity Commission regulations regarding affirmative action for individuals with disabilities in federal employment. These revisions also ensure that PS Form 2489 is consistent with the Office of Personnel Management’s self-disclosure form, SF-256, Self-Identification of Disability, which contains disability codes and descriptions for federal agencies.
Revised PS Form 2489 is now available on the Postal Service PolicyNet website:
