Effective November 8, 2018, the Postal Service™ revised PS Form 2181-D, Disclosure and Authorization for Consumer Reports and Investigative Consumer Reports, to update the following:
n A notice informing consumers of their right to a security freeze as required by the Economic Growth, Regulatory Relief, and Consumer Protection Act, signed into law in May 2018, and
n A sentence immediately preceding the security freeze notice clarifying that the right to security freeze applies only in regards to nationwide consumer reporting agencies.
Recycle all previous versions of PS Form 2181-D and begin using the current form immediately. Revised PS Form 2181-D is now available on the Postal Service PolicyNet website:
n Go to blue.usps.gov.
n In the left-hand column under “Essential Links,” click PolicyNet.
n Click Forms.
