Effective May 10, 2019, the Postal Service™ has revised PS Form 2574, Resignation/Transfer from the Postal Service, to reflect a change to the Human Resources Shared Service Center fax number used to submit resignation withdrawals.
The May 2019 edition of PS Form 2574 supersedes all previous editions and is available at usps.com, and on the Postal Service PolicyNet website:
n Go to blue.usps.gov.
n In the left-hand column under “Essential Links,” click PolicyNet.
n Go to the right-hand side under “Published Forms and Directives.”
n Click Forms.
