Effective November 2021, the Postal Service™ revised PS Form 1750, Employee Evaluation and/or Probationary Report, to include the replacement of “social security number (SSN)” to “employee identification number (EIN),” as well as updates to reflect the different probationary periods found within the various Collective Bargaining Agreements. The updated form is effective immediately.
The revised edition of PS Form 1750 is available on the PolicyNet website:

n In the left-hand column, click Essential Links, and then click PolicyNet.
n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 1000–1999.
n Scroll down to the form entry and click the PDF icon.
