Effective October 2021, the Postal Service™ published the following forms related to the Prevent All Cigarette Trafficking (PACT) Act of 2009 and to legislation enacted on December 27, 2020, that includes Electronic Nicotine Delivery Systems (ENDS):
n PS Form 4615, PACT Act Application for Business or Regulatory Purposes Exception — Cigarettes and Smokeless Tobacco.
n PS Form 4616, PACT Act Application for Consumer Testing or Public Health Exception.
n PS Form 4615-E, PACT Act Application for Business or Regulatory Purposes Exception — ENDS.
n PS Form 4615-EM, Mailer Information Worksheet for PS Form 4615-E (PACT Act — ENDS).
n PS Form 4615-ER, Recipient Information Worksheet for PS Form 4615-E (PACT Act — ENDS).
Note: The first two forms are revised editions and the last three are new forms. All previous editions of PS Form 4615 and PS Form 4616 are obsolete and must be recycled.
These forms will assist customers and the Postal Service during the application submission and review processes by having more consolidated and streamlined application data.
All these forms are dated October 2021 and are available online on the USPS® internal web and the external web at the following locations:
n Internal (blue.usps.gov): On the left, under “Essential Links,” click on PolicyNet; then on the right, click on Forms and the applicable range of form numbers; and then scroll down to the particular form and click on the link for it.
n External (usps.com): At the bottom of the page in the USPS.com® box, under “On About.USPS.com,” click on Forms & Publications; then click on Find a Form; and then scroll to the page with the particular form and click on the link for it.
Links to the online files are also available on Postal Explorer® at pe.usps.gov (in the “Domestic Mail” section, click on the box for PACT Act Information and Forms) and on PostalPro at postalpro.usps.com/product-classification/tobacco (scroll down to the “Featured Resources” section).
Note: PS Form 4615-EM and PS Form 4615-ER are associated with PS Form 4615-E. For these associated forms, the internal, external, and PostalPro sites include 1-page PDF files that show a “sample” of what the forms look like and the information they request. These PDF files have links — through the text “To submit a worksheet, click here for the Excel file” — to 1,000-line Excel worksheet files that users can download, complete, and submit with the actual application data. The Postal Explorer site does not have links to the PDF “sample” files of PS Form 4615-EM and PS Form 4615-ER, but just to the Excel worksheet files themselves. These Excel worksheet files might undergo revisions and might have revised dates and editions subsequent to October 2021.
