PS Form 2181-A Revision: Pre-Employment Screening — Authorization and Release

Effective January 2023, the Postal Service™ revised PS Form 2181-A, Pre-Employment Screening — Authorization and Release. The Privacy Act Statement has been updated to align with the Privacy Act System of Records (SOR) 7000.000, Inspection Service Investigative File System.

The use of revised PS Form 2181-A is effective immediately. All previous editions of PS Form 2181-A are obsolete and must be recycled.

Revised PS Form 2181-A is available on the PolicyNet website:

n Go to blue.usps.gov.

n In the left-hand column, click Essential Links, and then click PolicyNet.

n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 2000–2999.

n Scroll down to the form entry and click the PDF icon.
