With the exception of eVS and eInduction, mailers must present an original PS Form 8125 with a drop shipment and must accurately complete all required items in the Mailer Information section. The “Origin Post Office” section must be completed by an acceptance employee at the location where the mailing is verified and postage is paid. The driver may submit a second copy of the 8125, which once completed, will serve as their copy. In situations where load refusal may occur or issues cannot be easily resolved, it will be necessary to escalate the irregularity to a supervisor.
