Effective June 26, 2025, the Postal Service™ is revising PS Form 650, Request for ELM 650 Mediation, to add a box under section I, “Appellant/Requestor Information,” and also under section IV, “Representative Information,” to include an email address.
Additionally, revisions will include punctuation corrections and text changes for conciseness and to increase clarity.
Any new standards in the form will become effective immediately after the revised form has posted on blue.usps.gov.
