Effective September 4, 2025, the Postal Service™ revised the following forms:
n PS Form 5956-EXE, Executive Employee Probationary Period Report.
n PS Form 5956-MGR, Managerial Employee Probationary Period Report.
n PS Form 5956-SUP, Supervisor Employee Probationary Period Report.
The competency criteria on PS Form 5956-EXE, PS Form 5956-MGR, and PS Form 5956-SUP were updated to reflect revisions of the corresponding competency model published on MyHR. The updated form is effective immediately.
The online version of PS Form 5956-EXE, PS Form 5956-MGR, and PS Form 5956-SUP are available on the Postal Service PolicyNet website (blue.usps.gov/cpim):
n Go to blue.usps.gov.
n In the left-hand column under “Essential Links,” click PolicyNet.
n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 5000–5999.
n Scroll down to the form entry and click the PDF icon.
