PS Form 1750-P: Postal Support Employee Evaluation Report

Effective September 2026, the Postal Service™ created PS Form 1750-P, Postal Support Employee Evaluation Report. This new form has been added as an expansion of PS Form 1750 to correlate with the probationary period changes for postal support employees in the collective bargaining agreement. The updated form is effective immediately.

The online version of PS form 1750-P is available on the Postal Service PolicyNet website (blue.usps.gov/cpim):

n Go to blue.usps.gov.

n In the left-hand column under Essential Links, and then click PolicyNet.

n Go to the right-hand side under “Published Forms and Directives,” click Forms, and then click the range 1000–1999.

n Scroll down to the form entry and select PDF icon.

n The direct URL for the Postal Service PolicyNet website is blue.usps.gov/cpim.
