PS Form 650 Revision: Request for Mediation

Effective February 19, 2026, the Postal Service™ is revising PS Form 650, Request for ELM 650 Mediation, section V., to reflect a change in how and to whom applicants requesting mediation must submit the request.

This revised standard, which is expected to be published in PS Form 650 this month (February), is effective immediately.

[Revise the first two sentences of item 3. to read as follows:]

3) I understand that this form (or written request) and a copy of the discipline must be sent by email within ten (10) calendar days from my receipt of an applicable disciplinary action to the ELM 650 Mediation Facilitator, ELM650_Mediation_Request@usps.gov. The discipline must show the date I received it.***
