PS Form 1750-R: Employee Evaluation or Probationary Report

Effective March 20, 2025, the Postal Service™ is revising PS Form 1750-R, Noncareer Rural Carrier Employee Evaluation or Probationary Report. This new form outlines when to evaluate a noncareer rural carrier during their probationary period and provides instructions on how to complete the form. PS Form 1750-R also includes a Privacy Statement for compliance with the Privacy Act.

The updated version of PS Form 1750-R is effective immediately and available on the Postal Service PolicyNet website:

n Go to blue.usps.gov.

n In the left-hand column, click Essential Links, and then click PolicyNet.

n On the right-hand side under “Published Forms and Directives,” click Forms, and then click on the range 1000–1999.

n Scroll down to the form entry and select the PDF icon.
